PATH Program

The PATH program addresses chronically homeless individuals who suffer from mental illness. Services provided through the PATH program include:

  • Comprehensive healthcare
  • Mental health treatment
  • Case management
  • Substance use treatment
  • Referrals for shelter & affordable housing
  • Rehabilitation training in social skills & employment
  • Legal services
