Women in Transition (WIT)

CHC’s Women in Transition (WIT) program is here to assist women being released from York Correctional Institute in Niantic transition. 

Our highly trained Transition Case Managers help WIT clients connect with healthcare and other essential services. We can help clients connect to primary healthcare, behavioral healthcare, dental care, substance use support, transportation support, family reunification support, and more! 

If you know a woman scheduled to be released in the next 90 days from York Correctional Institute who could benefit from our services, please contact us at the information below.
